Case Management
APGD uses a centralized model of Case Management in the provision of services and has three Case Managers who assist clients to coordinate all bio-psycho-social services in a collaborative manner with the other local service providers. Case Management serves as the main information and referral source which links clients to services. Case Managers complete a comprehensive needs assessment at the time of intake and develop a plan based on each client's(s') unique needs and goals. Case Managers act as advocates for their clients and connect them with a variety of services such as: state, city, and federal assistance, medical treatment, transportation, education and counseling services, medical coverage, substance abuse treatment, in-patient care, housing or other needs as identified.
Counseling and Support Services
APGD facilitates an HIV Support Group for people living with HIV/AIDS and their loved ones every Tuesday night at 7:00 p.m. at the administrative office at 30 West Street. This group empowers clients to develop healthy networks, share experiences and learn more about HIV-related information in a safe, non-judgmental, confidential and compassionate setting. The APGD Mental Health Clinician is also available for individual, family or couples counseling. In addition, APGD hosts a variety of social gatherings for the clients throughout the year.
Transportation Services
The APGD Van is available for group transport during the Immunodeficiency Center Clinic Hours and the HIV Support Group. Case Managers can also help clients coordinate various modes of transportation as well as disburse tokens and fares for public transportation services.
Housing Opportunities
APGD can provide supportive living and scattered site housing opportunities for people living with HIV/AIDS. APGD has a Group Residence that can house up to five men and woman. Scattered site housing opportunities are available as a result of our state and federal partnerships. APGD Residential Staff work in conjunction with the Case Managers to assist clients locate and secure appropriate housing services.
Alternative/Complementary Therapies
Massage Therapy, Reflexology and Nutritional Counseling are available at the administrative office at 30 West Street. Please call for more information or to make an appointment for any of these services.
Emergency Financial Assistance
Emergency financial assistance may be available to assist with direct HIV-related care, housing expenses, utility bills, basic needs or other quality of life needs. A Case Manager can answer your questions regarding eligibility.
